PStart is an excellent program, but I find it to be cumbersome when you
have a lot of entries. Adding large amounts of programs to it takes too
many steps, and when run on a flash drive it takes forever to start up
as it loads all the icons and paths. It alsk takes a huge amount of RAM
when loaded, for such a small and simple program.

Quite by accident I discovered QSEL which seems to be a highly interesting
alternative ... though the interface is quite different (scrolling windows
of icons instead of a hierarchical start-menu like appearance). However,
adding new items is by drag-and-drop, it seems to load much faster than
PStart, and it is suitable for portable use because it figures out how to
use relative paths when run on removeable media.
